COVENIN 474:1997 standard

The Venezuelan standard COVENIN 474:1997"Recording, classification and statistics of work-related injuries" — establishes the methods for recording, classifying and computing workplace accident statistics in Venezuela. It was approved by FONDONORMA and applies to companies in any sector required to report industrial safety indices. SafeStats faithfully implements its formulas to compute IFN, IFB, IS and HHE.

How to compute accident statistics?
Step-by-step guide to the IFB, IFN and IS indices per the COVENIN 474 standard.
Definitions
HHE — Man-Hours of Exposure
Total hours worked by the entire workforce during the period. It is the common denominator of the indices.
NLT — Total Number of Injuries
Total number of work-related accidents recorded during the period (with or without lost time).
NLPT — Number of Lost-Time Injuries
Subset of accidents that cause at least one day of disability.
TDP — Total Lost Days
Sum of days actually lost due to all temporary disabilities in the period.
TDC — Total Charged Days
Sum of conventional days assigned per the COVENIN table for fatalities or permanent disabilities.
Formulas
HHE
(Workers × Working days × Daily shift) + Overtime hours − Lost hours
IFN — Net Frequency Index
(NLPT × 1.000.000) / HHE
Measures lost-time accidents per million man-hours.
IFB — Gross Frequency Index
(NLT × 1.000.000) / HHE
Measures total recorded accidents per million man-hours.
IS — Severity Index
((TDP + TDC) × 1.000.000) / HHE
Measures severity: lost and charged days per million man-hours.
Indicative table of charged days (TDC)
CategoryDescriptionCharged days
ITPDeath or total permanent disability6.000
IPPPartial permanent disability (reference; adjust per the specific injury table)1.500
